Elle TENS Rental Agreement

Thank you for choosing to rent an Elle TENS unit from New Spirit Arrivals.

Once you've submitted this rental agreement, you’ll receive an invoice for the $30 deposit, to complete reservation of a unit.

A receipt will be provided for those wanting to submit a claim to extended health benefits for reimbursement, after the unit has been return and deposit refunded.

The following rental agreement will take place between Rylie Geant of New Spirit Arrivals (the “owner”) and the individual named below (the “renter”).

Terms and Conditions

1. The Elle TENS will be used according to the instructions included along with the rental unit.
2. The TENS unit being rented is only meant to be used by the person whose name appears on this form and is non transferable.
3. The TENS unit will be tested before dispatch and will be supplied in good working order, ready for use and will be returned in the same condition received.
4. This rental includes: 1 Elle TENS unit, one package of electrodes (a set of 4), 2 sets of AA batteries (4 in total), 3 Leadwires, 1 easy release neck cord, Instruction Manual, Quick Start Guide, A Woman’s Guide to Drug Free Pain Relief (booklet), and a carrying pouch.
5. In the event that the TENS unit or any part thereof is lost, stolen or damaged in any way or becomes contaminated with bodily fluids, the renter will be charged the replacement value of $200.
6. The rental period is for a maximum of 5 weeks, beginning as early as 37 weeks gestation. Unless otherwise agreed upon with the owner, it is expected that the rental unit will be returned by the last day of the rental period. To extend the length of the rental, the renter must contact the owner and an additional $15 will be charged for each week or part thereof until the unit is returned. If the TENS unit is not returned for any reason the full replacement charge of $200 will apply.
7. TENS units are NOT compatible with water and should never come into contact with water or any fluids. If you intend to use the shower, bath or birthing pool, you must first turn off the TENS unit and remove the TENS pads.
8. It is the renter's responsibility to familiarize oneself with the TENS unit, the components and to read through the instruction manual. In the event that the renter discovers any issue whatsoever, the owner must be notified immediately. Failing to do so makes it impossible for the owner to take corrective action and so the owner takes no responsibility if not notified immediately.
9. All pieces of the original package MUST be returned, including expired or drained batteries, except for used electrodes. Used electrode can be disposed of.

Rental Fees
The Elle TENS rental fee is $65 (+ $30 deposit, which is refunded upon return of unit) for a maximum of 5 weeks (beginning at 37 weeks gestation) and includes 1 set of electrodes (4 pads). Every additional week, or part there of, is an additional $15.

Shipping is available for the added fee of $25.
(It will be mailed a minimum of 3 days before your 37th week to ensure it arrives on time.)

Cancellation & Refund Policy

1. If the rental agreement is cancelled prior to the rental period beginning, the renter will be refunded the rental fee less the deposit of $30 for reservation of the unit.

2. No refund will be made after the unit has been sent to the renter or for non-use of the unit.

Precautions & Contraindications

1. TENS machines contain electrical components and MUST NOT be used in or around water (or fluids). Remove the TENS and electrodes before entering a bath, the shower or birthing pool.

2. SHOULD NOT be used by persons with a cardiac pacemaker, any heart disease or epilepsy.

3. SHOULD NOT be applied internally or used on broken or cracked skin.

4. SHOULD NOT be placed over the heart or on the chest.

5. Keep the TENS unit and electrode pads out of reach of children and pets.

6. NOT to be used whilst pregnant before 37 weeks gestation without prior consultation and specific agreement from your care provider.

Waiver of Liability

1. I, the renter, DO NOT suffer from epilepsy, nor do I have a cardiac pacemaker or any suspected heart disease.

2. I, the renter WILL NOT hold New Spirit Arrivals responsible or liable for any injury or damage sustained as a result of misusing the TENS unit.
